There are two primary ways to get Red Mutagens in The Witcher 3. The first involves killing specific monsters and receiving the mutagen as a random loot drop. The second method is crafting Red Mutagens, a mechanic introduced at the release of the Blood and Wine expansion.

According to Fextralife, Archgriffins, Dopplers, Greater Foglets, Katakans, Nekker Warriors, Succubi, Water Hags, Werewolves, and Wyverns are good monster-drop sources of Red Mutagens in The Witcher 3. Remember that killing one of these beasts does not guarantee the mutagen; instead, there is but a good chance that you will find one on the mob’s corpse once it has been slain. Sources at Gamepur have added that Drowners and Ghouls can also drop Red Mutagens. A great place to farm Drowners would be the Whale Graveyard of Skellige, where many of these fiends spawn. Additionally, YouTuber Linus Wilson has discovered a method for farming higher-tier Red Mutagens with Monster Nests in Toussaint, but players will need to reach the DLC story for this route. 

Alternatively, you can craft Red Mutagens by combining other color Mutagens with specific ingredients. For instance, a Lesser Blue or Green Mutagen becomes a Red Mutagen when mixed with Rubedo and Winter Cherry. However, the formulas for crafting Mutagens are only available once Geralt has reached Toussaint of the Blood and Wine DLC.
